Full Description:
ONE ANESTHESIA
SIAC
CRNA/AA

• Hourly rate $74.52/hr
• Overtime $111.78/hr
• Base Salary $155,000
• Total package $190,000
• Paid on biweekly basis
• PTO 25 days
• Practice pays Licenses, DEA, Staff dues, and credentialing fees
• Benefit allowance $8,000 (to be used for health, vision, dental, H S A & FSA as employee wishes)
• Employer paid benefits $5,000/mo LTD and $50,000 group term life insurance
• CME allowance $1,500
• Malpractice Paid by Practice
• Company Profit Sharing eligible after 6 months; employer contributes 5% of compensation as profit sharing *3-year vesting; 0% vested years 1 & 2; fully vested after year 3
• Employer paid payroll taxes Employer pays social security, federal and state unemployment taxes as well as workers’ comp insurance
• Locations Baptist Health Floyd 1850 State St, New Albany Indiana
Physician Medical Center 4023 Reas Lane, New Albany Indiana
• Work schedule M-F 7-3:30 OR 7-4:30 with one rotating short day 7-11a; daily unpaid 30 minute lunch; no nights; no weekends; no call; no cardiac; no OB, no trauma, limited pediatrics
